All full-time freshman, graduate, and transfer students are required by Virginia State Law to submit health evaluation forms. These forms include a health history, childhood immunization records, and a Tuberculosis screening. Health records must be submitted prior to the start of your first semester. (Fall student records are due by June 1. Spring student records are due by December 1). Failure to submit these records will result in a health hold placed on your student account.
I have a health hold. What do I need to do to have it removed? Health holds are placed when health forms have not been received in a timely manner. Holds may also be placed when submitted records are incomplete or when your tetanus vaccine booster expires. Please contact the immunization clinic for specific information on what is needed to remove your health hold.
I am a Full-time/Graduate/Online/Non-traditional student. Do I still need to submit health forms? Yes, all full-time students, including those taking online classes, are required to submit the health forms. If you are a part-time student, please contact the immunization clinic about your enrollment status.

Do I need a physical? Currently, only student athletes are required to submit a physical. Athletes are required to submit a copy of their physicals to health services as well as their head coach and sports medicine.
Can I receive vaccinations on campus? We offer the tetanus and MMR vaccines as well as Tuberculosis skin tests (PPD) at Memorial hall. Please call to set up an appointment.

I am unable to locate my immunization records, what should I do?
Students who have incomplete records will need to repeat their vaccinations or get blood work known as “Titers” drawn to show immunity to specific diseases.
I do not wish to receive one or more vaccines. Can I submit a waiver?
There are waivers included in the health evaluation form packet for the Hepatitis B and Meningitis vaccine (page 15). All other required vaccinations can only be waived using a Medical Waiver signed by a health care professional or notarized religious waiver.
